Incidence and risk factors for clinically confirmed secondary bacterial infections in patients hospitalized for coronavirus disease 2019 (COVID-19)

Objective:
The true incidence and risk factors for secondary bacterial infections in coronavirus disease 2019 (COVID-19) remains poorly understood. Knowledge of risk factors for secondary infections in hospitalized patients with COVID-19 is necessary to optimally guide selective use of empiric antimicrobial therapy.
Design:Single-center retrospective cohort study of symptomatic inpatients admitted for COVID-19 from April 15, 2020, through June 30, 2021.
Setting:Academic quaternary-care referral center in Portland, Oregon.
Patients:The study included patients who were 18 years or older with a positive severe acute respiratory coronavirus virus 2 (SARS-CoV-2) PCR test up to 10 days prior to admission.
Methods:Secondary infections were identified based on clinical, radiographic, and microbiologic data. Logistic regression was used to identify risk factors for secondary infection. We also assessed mortality, length of stay, and empiric antibiotics among those with and without secondary infections.
Results:We identified 118 patients for inclusion; 31 (26.3%) had either culture-proven or possible secondary infections among hospitalized patients with COVID-19. Mortality was higher among patients with secondary infections (35.5%) compared to those without secondary infection (4.6%). Empiric antibiotic use on admission was high in both the secondary and no secondary infection groups at 71.0% and 48.3%, respectively.
Conclusions:The incidence of secondary bacterial infection was moderate among hospitalized patients with COVID-19. However, a higher proportion of patients received empiric antibiotics regardless of an identifiable secondary infection. Transfer from an outside hospital, baseline immunosuppressant use, and corticosteroid treatment were independent risk factors for secondary infection. Additional studies are needed to validate risk factors and best guide antimicrobial stewardship efforts.

We present the data and initial results from the first pilot survey of the Evolutionary Map of the Universe (EMU), observed at 944 MHz with the Australian Square Kilometre Array Pathfinder (ASKAP) telescope. The survey covers $270 \,\mathrm{deg}^2$ of an area covered by the Dark Energy Survey, reaching a depth of 25–30 $\mu\mathrm{Jy\ beam}^{-1}$ rms at a spatial resolution of $\sim$ 11–18 arcsec, resulting in a catalogue of $\sim$ 220 000 sources, of which $\sim$ 180 000 are single-component sources. Here we present the catalogue of single-component sources, together with (where available) optical and infrared cross-identifications, classifications, and redshifts. This survey explores a new region of parameter space compared to previous surveys. Specifically, the EMU Pilot Survey has a high density of sources, and also a high sensitivity to low surface brightness emission. These properties result in the detection of types of sources that were rarely seen in or absent from previous surveys. We present some of these new results here.
Do applications of systemic herbicides when green fruit are present prevent seed production or viability of garlic mustard (Alliaria petiolata)?

Garlic mustard [Alliaria petiolata (M. Bieb.) Cavara & Grande] is a biennial invasive plant commonly found in the northeastern and midwestern United States. Although it is not recommended to apply herbicides after flowering, land managers frequently desire to conduct management during this timing. We applied glyphosate and triclopyr (3% v/v and 1% v/v using 31.8% and 39.8% acid equivalent formulations, respectively) POST to established, second-year A. petiolata populations at three locations when petals were dehiscing and evaluated control, seed production, and seed viability. POST glyphosate applications at this timing provided 100% control of A. petiolata by 4 wk after treatment at all locations, whereas triclopyr efficacy was variable, providing 38% to 62% control. Seed production was only reduced at one location, with similar results regardless of treatment. Percent seed viability was also reduced, and when combined with reductions in seed production, resulted in a 71% to 99% reduction in number of viable seeds produced per plant regardless of treatment. While applications did not eliminate viable seed production, our findings indicate that glyphosate and triclopyr applied while petals are dehiscing is a viable alternative to cutting or hand pulling at this timing, as it substantially decreased viable A. petiolata seed production.

This is an epidemiological study of carbapenem-resistant Enterobacteriaceae (CRE) in Veterans’ Affairs medical centers (VAMCs). In 2017, almost 75% of VAMCs had at least 1 CRE case. We observed substantial geographic variability, with more cases in urban, complex facilities. This supports the benefit of tailoring infection control strategies to facility characteristics.
Midterm outcomes of pulmonary artery sling repair with and without tracheoplasty

Objective:
Review a single-centre experience with pulmonary artery sling repair and evaluate risk factors for re-intervention.
Methods:Patients with surgically repaired pulmonary artery sling at a single institution between 1996 and 2018 were retrospectively reviewed. A univariate Cox regression analysis was used to evaluate variables for association with freedom from re-intervention.
Results:Eighteen patients had pulmonary artery sling repair. At operation, median age and weight were 6.9 months (interquartile range 4.1–18.1) and 9.5 kg (interquartile range 6.5–14.5), respectively. A median hospital length of stay was 12 days (interquartile range 5.8–55.3). Twelve patients (67%) had complete tracheal rings, of whom six (50%) underwent tracheoplasty (five concurrently with pulmonary artery sling repair). Airway re-intervention was required in five (83%) of the six patients who underwent tracheoplasty. One patient had intraoperative diagnosis and repair of pulmonary artery sling during unrelated lesion repair and required tracheoplasty 24 days post-operatively. One patient died 55 days after pulmonary artery sling repair and tracheoplasty following multiple arrests and re-interventions. Median post-operative follow-up for surviving patients was 6.3 years (interquartile range 11 months–13 years), at which time freedom from re-intervention was 61%. When controlling for patient and tracheal size, initial tracheoplasty was associated with decreased freedom from re-intervention (hazard ratio 21.9, 95% confidence interval 1.7–284.3, p = 0.018).
Conclusions:In patients with pulmonary artery sling, tracheoplasty is associated with decreased freedom from re-intervention. In select patients with pulmonary artery sling and complete tracheal rings, conservative management without tracheoplasty is feasible. Further study is necessary to delineate objective indications for tracheoplasty.

Objective:
We examined Clostridioides difficile infection (CDI) prevention practices and their relationship with hospital-onset healthcare facility-associated CDI rates (CDI rates) in Veterans Affairs (VA) acute-care facilities.
Design:Cross-sectional study.
Methods:From January 2017 to February 2017, we conducted an electronic survey of CDI prevention practices and hospital characteristics in the VA. We linked survey data with CDI rate data for the period January 2015 to December 2016. We stratified facilities according to whether their overall CDI rate per 10,000 bed days of care was above or below the national VA mean CDI rate. We examined whether specific CDI prevention practices were associated with an increased risk of a CDI rate above the national VA mean CDI rate.
Results:All 126 facilities responded (100% response rate). Since implementing CDI prevention practices in July 2012, 60 of 123 facilities (49%) reported a decrease in CDI rates; 22 of 123 facilities (18%) reported an increase, and 41 of 123 (33%) reported no change. Facilities reporting an increase in the CDI rate (vs those reporting a decrease) after implementing prevention practices were 2.54 times more likely to have CDI rates that were above the national mean CDI rate. Whether a facility’s CDI rates were above or below the national mean CDI rate was not associated with self-reported cleaning practices, duration of contact precautions, availability of private rooms, or certification of infection preventionists in infection prevention.
Conclusions:We found considerable variation in CDI rates. We were unable to identify which particular CDI prevention practices (i.e., bundle components) were associated with lower CDI rates.

Drawing on a landscape analysis of existing data-sharing initiatives, in-depth interviews with expert stakeholders, and public deliberations with community advisory panels across the U.S., we describe features of the evolving medical information commons (MIC). We identify participant-centricity and trustworthiness as the most important features of an MIC and discuss the implications for those seeking to create a sustainable, useful, and widely available collection of linked resources for research and other purposes.

We reported a mammalian cell-imaging paradigm to study the cellular response to single-walled carbon nanotubes (SWCNTs). Chinese Hamster Ovarian (CHO) cells were exposed to SWCNTs resuspended in physiologically compatible buffer (phosphate buffered saline, PBS), at concentrations ranging from 0 to 50 μg/mL. Upon exposure, we optically imaged the cells in order to (1) visualize the accumulation SWCNTs in cells in real-time; (2) qualitatively and quantitatively assess the morphological changes associated with cellular stress in the presence of SWCNTs; and (3) serially quantify cell survival with highly sensitive bioluminescence-based imaging. Our results showed that the cell survival obtained from optical imaging agreed with that from CellTiter-Glo (CTG) luminescence viability assay. Acute compromise in the CHO cell’s survival rate was observed under high concentrations of SWCNT exposure. The cellular response as a function of SWCNT concentrations, and exposure time was further investigated.

The Tarantula Survey is an ESO Large Programme which has obtained multi-epoch spectroscopy of over 1,000 massive stars in the 30 Doradus region of the Large Magellanic Cloud. The assembled consortium will exploit these data to address a range of fundamental questions in both stellar and cluster evolution.
